Download a parental consent form re parish activities  and a Garda Vetting Form (for return to parish priest). If a person submitting a vetting form is under 18 and over 16, their Garda Vetting Form must also have attached to it a signed Garda Vetting Parental Consent Form. (People under 16 cannot be vetted)
